Documents Requirement for FSSAI Food License

The FSSAI required documents for Food License depends on what kind of registration you require like – Simple Registration Form A or Food License Form B. Whether you require Form A or Form B now depends on your annual turnover.For tiny FBO only registration with Food authority is required.

FSSAI Registration

    • Photo of the Food Business Operator.
    • Document for ID Proof. It may be Ration Card, Voter ID Card, PAN Card, Driving License, Passport, Aadhar Card, Senior Citizen Card, Department Issued ID.
    • Supporting Documents (if any):- NOC by Municipality/Panchayat, Health NOC.

    FSSAI State License

      IDs

      • 1. List of Directors or Members of Society/Trust with full address and contact details.
      • 2. list of Machinery along with their registration number, working capacity and power usage
      • 3. ID and address proof issued by Government of India for Authorised Signatory
      • 4. List of food category planned to manufacture.
      • 5. Copy & certificate obtained under Coop – 1861/Multi State Coop Act – 2002 in case of Cooperatives
      • 6. Declaration and Undertaking by FBO

      Legal Documents

      • 1. Form-B Duly completed and attested by the Proprietor or Partner or the Authorized member.
      • 2. Authority letter with name and address of the responsible person nominated by the manufacturer along with alternative responsible person.
      • 3. FSSAI Self-Declaration for Proprietorships
      • 4. NOC & Copy of License from manufacturer
      • 5. Food Safety Management System Plan or certificate.
      • 6. NOC from Municipality or local body.
      • 7. Form IX: Nomination of Persons by a Company alongwith the Board Resolution

        Optional

        1. Manufacturers

        • Analysis report of water to be used in food from a recognized health laboratory to confirm the potability
        • Proof of ownership or posession of premises.
        • Source of milk or acquisition plan for milk including location of milk collection centres (in case of Milk and Milk Products processing).
        • Sources of raw material for meat processing plants.
        • Pesticide residues report of water if the units manufacturing packaged drinking water, Mineral water and/or carbonated water from a recognized public health laboratory.

        2. Partnership

        • Partnership Deed or Affidavit of Proprietorship or Memorandum & Articles of Association towards the constitution of the firm.

        3. Transportation

        • Certificate provided by Ministry of Tourism.
        • Self declaration of number of vehicles.

        FSSAI Central License

          IDs

          • 1. List of Directors or Members of Society/Trust with full address and contact details.

          • 7. Blueprint of the manufacturing unit having the dimensions in metres/square metres and operation-wise area allocation.
          • 8. Supporting proof of turnover.

          • 3. FSSAI Self-Declaration for Proprietorships

          • 6. NOC from Municipality or local body.

          • 9. NOC documents issued by FSSAI.
          • 10. IE Code document issued by DGFT.
